The use of a photon sieve (PS) structure has been suggested to design intracorneal implants with drainage holes and multifocal properties. In this study we present how the customization of the 3D hole profile allows designing an innovative implant for the correction of presbyopia. This new implant consists in a transparent PS with bevelled holes whose use in a bilateral mix-and-match approach can provide good visual acuity on an extended range of distance. This blazed photon sieve is also apodized, enabling the ratio between energy in distance and near vision to be controlled according to pupil size. In addition, the hole rings of the photon sieve are placed on the odd radii of the Fresnel rings to maximize the surface area of the photon sieve slopes for small pupils, thus providing near vision energy for all pupil sizes.
